The difficulty is adjustable. A beginner sorting open tiles performs a simpler task than an experienced riichi player estimating waits, safety, score, and remaining tiles under time pressure. What studies have found

Several studies are encouraging, particularly among older Chinese adults, but they answer narrower questions than “Is Mahjong good for everyone’s brain?”

A 2006 exploratory study randomly assigned 62 older adults with mild-to-moderate dementia to play mahjong two or four times per week for 16 weeks. The authors reported gains across measured cognitive outcomes in both groups (PubMed). There was no non-mahjong control group in that comparison, the participants already knew how to play, and the sample was small.

A 2020 study randomized 56 nursing-home residents with mild cognitive impairment to a 12-week mahjong group or usual-activity control. The mahjong group improved on measures including executive function and functional activity (full paper at PubMed Central). It was a single-site study with 28 participants per arm, so replication in different populations matters.

A 2024 longitudinal analysis used Chinese Longitudinal Healthy Longevity Survey data from 2008–2018 and found an association between more frequent mahjong play and better cognitive trajectories (PubMed). Observational data can reveal patterns over time, but cannot fully remove selection effects: people who are healthier, more mobile, or more socially connected may also be more able to play often.

These findings support further research and make Mahjong a plausible engaging activity. They do not establish prevention, cure, or a universally effective “dose.”

Mental exercise is not a medical treatment

Memory complaints have many possible causes, including sleep problems, medication effects, mood, hearing loss, metabolic conditions, and neurological disease. A tile game cannot identify the cause. Seek qualified medical advice for new or worsening confusion, changes in daily function, safety concerns, or distress.

Do not pressure a person with cognitive impairment to play because it is supposedly therapeutic. The activity should be voluntary, adapted, and enjoyable. Structured programs for people with dementia need staff or family who understand fatigue, behavior changes, fall risks, and the person’s care plan.

Mahjong also should not displace movement, sleep, medical appointments, or existing relationships. Its strength is that it can combine thought and conversation in one repeatable leisure ritual.

How to play with realistic goals

For general enrichment, choose a variant whose challenge matches the group. Use open hands while learning, then introduce calls and scoring gradually. A one-page reference reduces avoidable memory load while leaving actual decisions to the player.

Vary the task without turning it into a test:

  • Sort a mixed set by suit and rank.
  • Build several different waits from the same tiles.
  • Name every visible discard before acting.
  • Explain one decision after the hand, without criticizing the result.
  • Alternate in-person social sessions with short solo practice.

Stop when concentration falls or mistakes become upsetting. There is no evidence that pushing through exhaustion creates a better brain outcome.

Social benefits may be the most immediate

Many players do not need a laboratory score to notice that a regular table creates anticipation, conversation, and belonging. A scheduled game can bring people into the same room and give newcomers a structured reason to speak. Those outcomes matter even if a player never becomes strategically advanced.

Online Mahjong can provide competition and access when geography is a barrier. Face-to-face play adds physical tiles, shared meals, and richer nonverbal interaction. Current evidence does not let us rank one format as clinically superior.
